PARTICIPATION AGREEMENT — Mini Volley 1. Who I am. I am the parent or legal guardian of every player named on this registration, and I have the authority to sign this agreement on their behalf. 2. What the program is, and who runs it. Mini Volley is a coached youth volleyball program for children aged 6–12, held in school gyms, colleges and community centers. Sessions run about 75 minutes, once a week. Players run, jump, dive for the ball, and share a court with other children. Mini Volley is run by Forza Volleyball and Boston United Volleyball Club, Inc. (together, the "Club"). Where this agreement says "Released Parties", it means the Club and their respective officers, directors, coaches, staff, volunteers, independent contractors, and agents. 3. The risks I am accepting. Volleyball carries a risk of injury that no amount of coaching removes. That includes sprains and strains, broken bones, collisions with other players, the floor, the net or the posts, being struck by a ball, and — less often but really — serious injury including concussion. I also accept the ordinary risks of getting to and from a session and of being in a shared public building, including catching something. I acknowledge that I have read this entire agreement, that I understand its terms, that I have had the opportunity to ask questions before signing, and that I am signing it freely and voluntarily. I understand that by signing I am giving up substantial legal rights, including the right to sue the Released Parties for their ordinary negligence. Additionally, I understand that these risks exist whether or not they are described above, that they include risks that are not foreseeable, and that I voluntarily assume all such risks on behalf of my child. I have had the opportunity to observe or inquire about the program, its coaches, and the facilities used, and I am satisfied that the program is suitable for my child. I acknowledge that the Club does not own, operate, maintain, or inspect the schools, colleges, or community centers where sessions are held, and that the Club makes no representations or warranties regarding the condition or safety of those facilities. 4. Indemnification. To the fullest extent permitted by Massachusetts law, I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, demands, actions, damages, losses, costs, and expenses (including reasonable attorneys' fees and court costs) arising out of or related to my child's participation in Mini Volley, including any claim brought by or on behalf of my child, except to the extent such claim is caused by the gross negligence of the Released Parties. 5. My child is fit to play. So far as I know, each player is in good health and able to take part. I have written on this form any medical condition, allergy, medication or current injury a coach would need to know about in a hurry, and I will tell the Club in writing if that changes during the season. The Club is entitled to rely on the completeness and accuracy of the medical information I provide. I assume all risk arising from any failure to disclose or update a medical condition, allergy, medication, or injury, and I agree that such failure shall not form the basis of any claim against the Released Parties. 6. Emergency medical care. If a player is hurt or becomes unwell and I cannot be reached, I authorize Club staff to give or arrange first aid, to call emergency services, and to consent to emergency medical or dental treatment recommended by a licensed provider. I am responsible for the cost of that treatment and of any ambulance. The Club does not provide accident or health insurance for players. 7. Head injuries. Any player showing signs of a concussion is taken out of play for the rest of that session, and does not return to Mini Volley until a licensed healthcare provider clears them in writing. 8. Release of claims. To the fullest extent Massachusetts law allows, I release, and agree not to sue, the Released Parties, and the schools, colleges and community centers whose facilities we use, for any claim arising out of my child's participation in Mini Volley — including claims based on ordinary negligence. This release does not cover gross negligence, reckless conduct, or intentional or criminal acts, and nothing in it gives up a right that Massachusetts law does not allow a parent to give up on a child's behalf. 9. Conduct. I agree that my child, and the adults who bring them, will treat coaches, other families and facility staff with respect. The Club may remove a player or a spectator from a session for behavior that makes the gym unsafe or unwelcoming. 10. Photographs are separate. Permission to photograph or film your child is asked for separately, below. It is optional, it is not part of this agreement, and your child's place in the program does not depend on it. 11. The rest. This agreement covers the Fall 2026 season and is governed by Massachusetts law. If a court finds any part of it unenforceable, the rest still stands. I am signing by typing my name below. I agree that my typed signature below constitutes an "electronic signature" as defined by the Massachusetts Uniform Electronic Transactions Act (M.G.L. c. 110G) and the federal Electronic Signatures in Global and National Commerce Act (E-SIGN Act, 15 U.S.C. § 7001 et seq.), and that it carries the same legal force and effect as a handwritten signature.

Photos and video — optional

PHOTOS AND VIDEO — optional, and your answer does not affect your child's place. We photograph and film sessions and the end-of-season Festival, and we use what we take to show families what Mini Volley actually looks like — on our websites, in emails to families, on social media, and in printed flyers. If you tick the box, you give Forza Volleyball permission to use images and video of your child for those purposes, with no payment, and you agree we may go on using anything already published. We never put a child's last name, school or contact details next to their picture. If you leave it unticked, nothing else changes. Your child's name goes on a do-not-photograph list that every coach in your town is shown, and we will not knowingly use their image. You can change your mind in either direction by emailing nikita@bostonuvc.com.

Leaving this unticked will not affect your child's place.

Every town needs 8 kids signed up by September 7 to run this fall. If yours doesn't get there we'll look at combining it with a nearby town, and if the new time or place doesn't work for your family, you get a full refund, no questions asked.
